Job Description

Elevate Support Care NSW Pty Ltd is looking for a caring and committed Community Worker to assist individuals, families, and community groups in accessing the support they need. This role involves identifying client needs, coordinating suitable services, promoting personal wellbeing, and encouraging active community participation. The successful candidate will build trusted relationships with clients, service providers, and local organisations to deliver positive and meaningful outcomes.

Tasks & Responsibilities
  • Assist individuals and families in identifying their needs, goals, and available support options.
  • Prepare and implement personalised support plans based on individual circumstances.
  • Refer and connect clients to appropriate community services, programs, and resources.
  • Organise and deliver community activities, educational workshops, and outreach initiatives.
  • Provide clients with information, practical assistance, and ongoing guidance.
  • Maintain complete and accurate client files, case notes, assessments, and progress reports.
  • Represent and advocate for clients when communicating with relevant services and agencies.
  • Build professional relationships with government departments, community organisations, and other service providers.
  • Encourage social inclusion, independence, diversity, and meaningful community involvement.
  • Follow organisational policies, privacy requirements, and professional standards at all times.
Qualifications and Skills
  • Certificate, Diploma, or Degree in Community Services, Social Work, Human Services, or another relevant discipline.
  • Previous experience in community services, support work, case support, or a related position is desirable.
  • Strong verbal and written communication skills.
  • Ability to establish respectful and professional relationships with clients and stakeholders.
  • Confidence working with individuals from different cultural and social backgrounds.
  • Strong planning, organisation, and time-management abilities.
  • Effective problem-solving skills and the ability to respond appropriately to individual needs.
  • Understanding of community programs, referral pathways, and local support services.
  • Ability to manage sensitive information while maintaining privacy, confidentiality, and professional boundaries.
  • Ability to work both independently and collaboratively as part of a multidisciplinary team
